All information held about patients is completely confidential.

The practice is registered under the Data Protection Act 1984. This act protects data held on the practice computer system.

TEACHING

As a teaching practice, medical students spend part of their training with us from Derby Medical School. Student nurses are also sometimes present. We would value your co-operation, but would understand if you did not wish to be involved. Please inform Reception.

COMPLAINTS & SUGGESTIONS

pen12 We always aim to provide you with the best service possible and welcome any suggestions on how this may be improved, verbally or written. However should you wish to complain about any practice procedures or members of staff, please ask Reception for the relevant form to fill in.

If for any reason you do not find this procedure helpful you can also contact the patient liaison service (PALS) 0845 6024 726. There is also an information leaflet concerning this and any other comments you wish to make available at reception.

ZERO TOLERANCE POLICY

Inappropriate or unpleasant behaviour will not be tolerated at the surgery. Details held at reception.

Travel vaccines

Please let us know well in advance if you are planning a holiday and think you may require travel vaccines. All travel vaccines have to be paid for prior to the Nurse appointment.
