Are you a commuter?
Patients may now register at The Surgery regardless of home address, for more information on the services available please contact reception.
How to Register as a Patient
We now offer an online GP Registration Service for new patients.
- If you are new to the area and wish to register with this practice, you can, of course, still pop in and ask at reception
- You will be issued with a welcome pack from our Reception team and included in that will be an option for you to book a well man/woman check with our practice nurse should you wish to do so
- Acceptance on to the list is as at the discretion of the Doctor and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this).
There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area. Please note that your medical records may take several weeks to arrive at the Practice.
Proxy Access

Practice Area
Our practice area denotes the boundary for the Home Visiting Service for those patients incapacitated by illness or disability. Patients outside our practice area may still register at The Surgery for convenience, such as commuters who find it easier to attend a Health Centre throughout the working day and journey. Patients wishing to register from outside the boundary are not eligible for our Home Visiting Services, those will be undertaken by the 111 Service if ever necessary. For more information please call us to discuss the full remit.
Catchment area
We can only register patients who currently live within our catchment area. Please enter your postcode below to check if your address is within our area.
The National Care Record Service (NCRS)
Important information regarding your NHS medical records – Summary Care Record
The National Care Record Service (NCRS) is the way in which health professionals now access the Summary Care Record (SCR). The SCR has not changed, it is just the way SCRs are accessed that has changed.
For more information about NCRS please see: National Care Records Service
Information for patients who wish to opt out of having a summary care record, including an opt out form is available here: Opting out
Dispensary
The dispensary is for patients who live more than 1 mile from a retail pharmacy. If you believe you fulfill this criteria (which is set by the government), please let us know as the practice is able to dispense most medications prescribed before you leave the surgery (saving any delay or need to visit a pharmacy).
The dispensary accepts telephone requests for repeat prescriptions from elderly and housebound patient only.
